Individual and Family Health Care Options Include:
- Pearl 25: An HMO plan, with no deductible and a minimal copay for most benefits and offers. This plan covers office visits, preventative medicine, hospitalization, and prescription drugs (with a one hundred dollar deductible).
- Diamond 15: A PPO plan which allows members to choose a Primary Care Physician who is in-network or non-network, with a 20% copay for the former and a 50% copay for the latter, and which offers a range of premiums ($250 – $7,500) and copay options.
- Emerald 40 PPO: Emerald 40 offers a higher deductible and lower premiums than the Diamond 15 plan.
- Garnet 50% PPO: This plan offers a choice of a $10,000 deductible or of no deductible, and a 50% copay for both in-network and non-network physicians.
- Crystal HDHP: This PPO plan is designed for use with your HSA account. With deductibles ranging between $1,000 and $7,000 (depending on your choice of provider), members have lower premiums.
